Description

The RN/LPN Case Manager provides clinical support and performs patient care coordination activities for the company. The RN/LPN Case Manager facilitates patient care services through ongoing communication with clinic treating staff, referral sources, acute care facilities, primary care providers, home health agencies, medical suppliers and others involved in the patients treatment plan.

No nights or weekends

Job Requirements

CASE MANAGER / RN or LPN

-Licensed RN or LPN
-CPR
-Must have previous wound care experience
