DUTIES:Back to top You will be responsible for identification and management of soldiers who may be medically non-deployable. Participate in joint planning with the health care provider and team, health benefits advisors, SRP referral coordinators, ambulatory clinic case managers, to coordinate workup and care of potentially non-deployable soldiers, civilians or contractors. Help identify appropriate provider and facilities throughout the continuum of services while ensuring that available resources are being used in a timely and cost effective manner. Develop, coordinate, implement and evaluate the efficacy of the individualized case management plan. Monitor soldiers over time to ensure progress is appropriate and recommends corrective action as required. The evaluation process involves continuous monitoring and analysis of the patients response to the services provided and timely intervention and re-planning when changes or problems with the patient occur. Utilize operating procedures, such as SRP standard operating procedures for both Active Duty and Reserve Component soldiers and evaluates outcomes and effectiveness. Facilitates communication and coordinates medical services between all members of the health care team and medical facilities to optimize medical readiness and improve coordination with unit commanders. Educate the patient and all members of the health care delivery team on case management issues, community resources, or other issues that may affect the health care teams or the patients decision. Educates the patient on the appropriate use of medical facilities and services, resulting in improve quality of care and medical readiness. There is a strong emphasis placed on educating patients on deployment and fitness for duty issues, as well as proper follow-up processes. Work in close collaboration with physicians and other health care personnel in the evaluation and disposition of the patient, including assisting in the U.S. Central Command (CENTCOM) waiver process. Identifies soldiers with unresolved issues and provides counseling and continued consultative support to patients regarding their deployability and action required to achieve desired timelines for deployment. Degree: Applicants MUST have a degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant. Licensure/Certification: Applicants MUST have active, current registration as a professional nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Specialized Experience: You MUST clearly demonstrate in their resume that they possess one (1) year of specialized experience at the next lower level, as a Registered Nurse, independently performing a variety of complex assignments involving collection, analysis and validation of a variety of data to determine the effective utilization of medical resources; quality assurance, and management criteria are met; identification of positive and negative patterns and trends; resolution of identified problems and recommendation for program improvement. Only degrees from an accredited college or university recognized by the Department of Education are acceptable to meet positive education requirements or to substitute education for experience.
